5TH GRADE YEARBOOK DEDICATIONS

Make your child’s 5th grade yearbook even more special with a yearbook dedication! This is a great way to tell them how proud you are and celebrate how much they’ve grown as they prepare for their next step in school.

This year, the Saigling PTA is excited to offer several yearbook dedication templates to choose from—or you may create your own design using any program you like. Want something extra special? For an additional $15 per dedication, our PTA team is happy to create a custom design just for your child.

2025-2026 Yearbook Dedications - Google Form

To complete your yearbook dedication order, please be sure to:

1. Select your dedication size from the options below and complete your purchase.  If you are requesting a custom design (outside of the provided templates), please remember to also add the “Custom Design Upcharge” to your order.

2. Complete the Google Form to submit your personalization details or upload your own design.

Don't forget to tell us which student this is for when prompted.

Once you have placed your order, you will recieve an email with the Google Form link again to choose your template and other personalization details. You must purchase your yearbook dedication on MTK AND complete the Google Form to fully submit your design.
